What tips do you have for identifying when I’ve entered a state of unconsciousness and self-assault?

WHen no important work isn’t being done. When I feel sorry for myself that I am not able to perform a task . When i miss my phone for no real reason. When feel sleepy an hour after I woke up and as I think of the task I’m about to start. When I try to make a todo list with things I have not bothered to check for over a week.
