For as long as barbell culture has kept a written record, it has repeated one rule: to build muscle, eat more than you burn. But the rule always traveled with footnotes. "Except if you're new to training, then you can build muscle in a deficit," says host Jordan Feigenbaum on the Barbell Medicine podcast. "And except if you're carrying extra body fat, then you can maximize building muscle in a deficit, too" (00:01). Sports scientist and competitive bodybuilder Eric Helms directs the Sports Performance Research Institute at Auckland University of Technology in New Zealand, and he has spent roughly a decade trying to explain why the exceptions kept eating the rule alive.

Host Jordan Feigenbaum opens with the sharper version of the question. Not whether fat turns into muscle, which it does not, but whether the energy stored in body fat can pay the metabolic bill for building new muscle tissue.

To answer that, the two of them first tighten some definitions. Hypertrophy is the growth of existing muscle fibers, measurable by ultrasound, DEXA scan (a body-composition scan that estimates fat, lean mass, and bone), or muscle biopsy (11:53). Maintenance calorie intake sounds like the level where body weight holds steady, but Helms argues that is a shortcut that breaks under scrutiny. A person who gains a kilogram of muscle while losing a kilogram of fat looks weight-stable on a scale, yet they were technically running a deficit the whole time, because fat carries far more stored energy per kilogram than muscle does.

Helms likes to think of the body as running two accounts: body fat as a savings account, and muscle and glycogen as a checking account. A surplus or a deficit is really a statement about which account the body is drawing from or adding to over time, not a statement about what the scale says on a given morning (14:42). That reframing matters because it lets the conversation get specific about cost. A recent modeling preprint by Nikolaidis and colleagues, cited by Helms, estimates the energetic cost of building one kilogram of muscle tissue at roughly 3,200 to 3,700 calories, once synthesis, deposition, and maintenance of the new tissue are all counted (44:05). That sounds like a lot until it gets divided across a month. If someone is gaining muscle at a rate of one kilogram per month, that works out to only about 115 extra calories a day (54:46), an amount that most people carrying meaningful body fat already have sitting in storage.

Where the ceiling actually sits

Energy is not the only constraint, though. Helms spends real time on what actually sets the pace of muscle growth in the first place, and lands on training stimulus rather than a fixed biological ceiling. University-aged men in typical hypertrophy studies gain about one to three kilograms of lean tissue in their first eight to twelve weeks of training (19:51), and the minimum effective dose to produce measurable hypertrophy in a lab is roughly four sets per muscle group, taken close to failure, once a week (27:39). Push the stimulus higher, and the ceiling rises with it. That framing sets up a warning about the second half of the surplus argument: adding calories without adding stimulus does not build more muscle, it just builds fat. A 2013 study by Garthe and colleagues split elite Norwegian athletes into two groups, one eating an intentional 500-calorie surplus, the other eating habitually. The surplus group ended up roughly 600 calories ahead of the other group on average, but the extra size was almost entirely fat. Their jump height and sprint times actually got worse (52:20 to 53:23).

A larger meta-regression by Murphy and colleagues, covering mostly untrained or lightly trained participants who were also carrying extra body fat, found the inflection point for losing muscle sat around a 500-calorie-per-day deficit. Inside a deficit of roughly 100 to 300 calories a day, body recomposition, meaning simultaneous fat loss and muscle gain, was common (66:21 to 66:52).

The two poles of body fat

The picture changes at the extremes of leanness. Helms points to research he co-authored with Ruffolo and Trexler, a meta-regression that split lifters into body-fat tiers. Below roughly 12% body fat for men (add about 8 percentage points for a rough female equivalent), the protein intake needed to hold onto muscle while dieting jumped to about 2.5 grams per kilogram of body weight, compared with about 1.6 grams per kilogram for lifters above 20% body fat (59:16 to 65:34). The mechanism traces back to old total-starvation research: in lean subjects, short-term fasting roughly doubles or triples leucine oxidation and gluconeogenesis, meaning the body starts burning protein for fuel and pulling glucose from non-carbohydrate sources at a much higher rate (68:12). Fat, in other words, stops being available as a savings account once there isn't much of it left.

A study by Vargas-Molina and colleagues tested this directly. Late-stage novice lifters, most of them men averaging around 20% body fat, were split into a deficit group and a near-maintenance group over eight weeks. Both groups gained about one kilogram of lean mass; the deficit group simply lost more fat to make up the energy gap (76:47 to 80:06). Above that body-fat threshold, in other words, the deficit wasn't the limiting factor at all.

Real-world sport backs up the pattern. Weight-class powerlifters who get very lean chasing a lower class, such as Taylor Atwood, often see stalled totals that resolve almost immediately after moving up a class, once the body is no longer forced to defend a body-fat floor (85:36 to 86:26). Sumo wrestlers, meanwhile, carry unusually high amounts of lean mass for their size, which Helms attributes to constant heavy mechanical loading, roughly analogous to living under artificially higher gravity (106:35). And leanness costs are not only cosmetic: in the SEMILEAN trial, people with obesity who lost more than 15% of their body weight on semaglutide, without an exercise program, still saw grip strength improve even as total lean mass fell (111:07), a reminder that muscle mass and muscle function are not the same measurement. Extreme deficits carry their own separate risk regardless of starting body fat: even in people with obesity, aggressive calorie restriction can disrupt the pulsing release of luteinizing hormone, an early hormonal warning sign tied to reproductive and bone health (117:00).

What Helms lands on, after ten years of turning the question over, is not a single number but a range. Above about 20% body fat, a man can run a deficit of up to roughly 400 calories a day and still build maximum muscle, because his body will spend fat stores to cover the gap. Below about 12%, that willingness to spend disappears, and a small surplus starts to matter. Everywhere in between, which is where most active adults actually live, calorie balance barely moves the needle, and training load becomes the real lever (122:36 to 123:46). Asked what would prove him wrong, Helms doesn't dodge the question: he'd want to see the Vargas-Molina result fail to replicate, or see meta-regressions show a true nonlinear jump in muscle gain right at maintenance. So far, he says, nothing has clearly broken the model (121:12). That is a modest claim for ten years of thinking, and it is exactly the kind of claim worth trusting.
